Occupational Therapist (OT)

Location: Routt or Moffat Counties

Required: Reside in CO or have plans to relocate

Job Description:

​Blue Skies Nursing is looking for part-time and full-time CNAs to provide high-quality, one-on-one home nursing care for our clients in the comfort of their homes.

Key Responsibilities:

• Deliver in-home patient care for all ages based on individualized care plans.
• Assess and address patient care needs, including the home environment for safety, recommend modifications, and teach strategies and techniques to manage daily tasks. This includes self-care, household management, and community reintegration.
• Monitor patient progress in terms of ability to perform daily activities and maintain independence, address caregiver concerns, and update physicians and Blue Skies Nursing supervisors as needed.
• Collaborate with healthcare professionals to optimize care plans and provide recommendations for enhanced healthcare

What You’ll Bring:

• Qualifications: Active OT license, CPR certification
• Experience: Previous experience in a similar role or relevant training.
• Skills: Strong assessment, problem-solving, and communication skills.
• Traits: Compassionate, resilient, and adaptable to varying patient needs.
• Requirements: Reliable transportation, auto insurance and a valid driver’s license.
