Ahava Therapy Services is seeking a PRN OT to join our in-house rehabilitation team at Wexford House, a 173-bed skilled nursing facility offering short- and long-term rehabilitation.

It's praised for its low traffic, family-friendly atmosphere, four seasons, and convenient location in the Tri-Cities region.

✅ Responsibilities

• Review clinical information and evaluate its impact on the assessment and treatment process.
• Conduct comprehensive functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable functional goals.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
• Revise treatment plans based on patient performance and progress.
• Supervise and guide therapy assistants, ensuring proper treatment techniques and care delivery.
• Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover.
• Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
• Assist with discharge planning and stay current on community resources.
• Participate in care conferences, meetings, and quality improvement initiatives.
• Maintain clinical competence through continuing education opportunities.
• Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.
• Support the company’s mission, vision, and professional expectations.
• Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.

✔️ Qualifications

• Bachelor’s or Master’s Degree in Occupational Therapy
• Active OT license in the state of practice
• BLS/CPR certification (current or obtained during orientation)
• Strong clinical reasoning, communication, and documentation skills
• Ability to collaborate effectively within an interdisciplinary team
