Welcome Home Health LLC is currently seeking a compassionate and dependable Occupational Therapy Assistant (OTA) to join our growing home health team. We are a Medicare & Medicaid-certified agency proudly serving patients throughout Palm Beach, Martin, St. Lucie, and surrounding counties.

Position Summary

The Occupational Therapy Assistant (OTA) is responsible for delivering skilled occupational therapy services in patients' homes under the direction and supervision of a licensed Occupational Therapist (OT), in accordance with the plan of care. The OTA implements treatment interventions, assists patients with regaining independence in daily activities, educates patients and families, and documents progress toward established goals.

Responsibilities

• Provide skilled OT interventions per the plan of care established by the supervising OT for Start of Care, Resumption of Care, Recertification, and Discharge episodes
• Assist patients with Activities of Daily Living (ADLs) such as dressing, grooming, feeding, bathing, and functional mobility to promote maximum independence
• Implement therapeutic exercise programs, adaptive equipment training, home safety education, energy conservation techniques, and fine motor and cognitive-perceptual interventions
• Serve pediatric, adult, and geriatric patients, including those with neurodevelopmental disorders, orthopedic conditions, stroke, or age-related decline
• Educate patients and caregivers on safety, fall prevention, adaptive equipment use, and home exercise programs
• Coordinate care with the supervising OT, physicians, PT, ST, RN case managers, and other members of the interdisciplinary team
• Report changes in patient status promptly to the supervising OT
• Ensure compliance with Medicare, Medicaid, and agency guidelines, including supervision requirements for OTAs
• Complete all documentation accurately and timely in the EMR system

Qualifications

• Active Florida OTA License (Board of Occupational Therapy Practice)
• Current NBCOT Certified Occupational Therapy Assistant (COTA) credential preferred
• Graduate of an ACOTE-accredited OTA program
• Minimum 1 year of OTA experience preferred
• Home health experience preferred
• Valid Florida driver's license and reliable transportation
• Current CPR Certification
• Strong communication and organizational skills
• Experience with EMR documentation preferred
• Bilingual (English/Creole or English/Spanish) is a plus
