JOB SUMMARY:

• Evaluate/assess the needs of referred patients in the outpatient, acute and hope unit areas

to formulate treatment plans

• Offer effective, patient centered services that will assist patients to return with confidence

to as much independence as possible with daily tasks and activities

• Organize and conduct a medically prescribed occupational therapy program to restore

function and prevent disability following disease, injury, or loss of body part

• Help all patients reach their maximum performance and assume a place in society while

learning to live withing the limits of their capabilities
DUTIES AND RESPONSIBILITIES:

• Promote the mission, vision, and values of the organization
• Provide therapy services defined in treatment plans
• Work cooperatively with physicians, case managers, and other members of the healthcare

team

• Maintain accurate, timely documentation of therapy services in the medical record
• Serve as clinical mentor and/or instructor to colleagues
• Assist with program development and performance improvement for the department
• Supervise Occupational Therapy Assistants, therapy techs and students as needed
• Perform other related duties as assigned

EDUCATION, EXPERIENCE, CERTIFICATIONS AND LICENSING REQUIREMENTS:

• Graduate of an accredited Occupational Therapy Program (minimum of bachelor’s

degree)

• Current Louisiana State Occupational Therapy license (temporary license allowed)
• BLS Certification

PHYSICAL REQUIREMENTS:

• Must be able to transfer patients
• Must be able to stand on your feet for extended periods of time

SAFETY PRACTICES:

• Consistently follows all established Safety procedures, incident/accident protocol and

precautions or universal precautions when performing duties relating to work area.

• Always follows good infection control practices and is familiar with Infection control

policies, procedures, PPE guidelines and guidance relevant to area of work.

• Practices proper body mechanics and ergonomics at all times.
• Follows established plans, policies, and procedures during emergencies, such as fire

evacuation, fire drills and actively participates in emergency exercises/disasters etc.

• Reports all Life Safety maintenance issues, defective equipment, etc. through the

Environmental Services work order system.
