Occupational Therapist (OT) – PRN The Elms Center | Milford, NH $50–$55/hour | PRN Stern Therapy Consultants is looking for a PRN Occupational Therapist at The Elms Center in Milford, NH . Use your clinical skills to help skilled nursing residents rebuild daily routines, adjust to changing abilities, and work toward meaningful goals during rehabilitation.

What You'll Do

• Assess how strength, coordination, cognition, and endurance affect residents' ability to manage everyday tasks.
• Establish individualized plans of care with measurable goals for self-care and functional participation.
• Deliver treatment through practical activities such as dressing, grooming, bathing, and toileting.
• Identify adaptive equipment and environmental modifications that support safer task performance.
• Educate residents and caregivers on strategies they can use between sessions and after discharge.
• Review progress, oversee COTA services within the treatment plan, and communicate recommendations through documentation and care team discussions.

Qualifications
