Outpatient Medical Coders | OP Coders | Outpatient Coding

We are looking for Outpatient Medical Coders to join our growing Healthcare BPO team. This role is responsible for reviewing outpatient medical records and assigning accurate diagnosis and procedure codes in accordance with official coding guidelines, documentation, and client requirements.

Key Responsibilities

• Review outpatient medical records, physician notes, diagnostic reports, and supporting clinical documentation.
• Assign appropriate ICD-10-CM, CPT, and HCPCS codes based on documentation.
• Apply current outpatient coding guidelines, conventions, and payer requirements.
• Ensure all assigned codes are supported by appropriate clinical documentation.
• Review charts for completeness, accuracy, and coding compliance.
• Identify documentation inconsistencies and escalate issues through the appropriate channels.
• Abstract relevant patient and clinical information into designated systems.
• Maintain established productivity, accuracy, quality, and turnaround-time targets.
• Participate in coding audits, quality reviews, and calibration sessions.
• Research coding guidelines and stay updated on changes to coding standards and regulations.
• Assist with identifying coding errors and opportunities for process improvement.
• Maintain strict confidentiality when handling patient records and protected health information (PHI).
• Collaborate with Quality, Operations, Clinical Documentation, and other support teams as required.

Qualification

• 1+ year of outpatient medical coding experience preferred.
• Strong knowledge of ICD-10-CM, CPT, and HCPCS coding systems.
• Working knowledge of outpatient coding guidelines and conventions.
• Strong understanding of medical terminology, anatomy, physiology, and disease processes.
• Ability to accurately interpret physician and clinical documentation.
• Excellent attention to detail and analytical skills.
• Strong written and verbal English communication skills.
• Proficient in computer applications and electronic medical record systems.
• Ability to work independently and meet productivity and quality targets.
• Strong organizational and time-management skills.
• Willing to work on a night shift or shifting schedule, depending on business requirements.
