Job Summary

Mindful Recovery Center is seeking a motivated and community-focused Outreach Coordinator / Case Manager to serve as a liaison between our behavioral health programs, clients, families, and community partners. This position is responsible for developing referral relationships, increasing community awareness of our services, coordinating resources for clients, and supporting program growth. The ideal candidate will have knowledge of Maryland behavioral health services, COMAR regulations, HIPAA, Medicaid requirements, and community-based resources.

Duties

• Build and maintain relationships with hospitals, schools, healthcare providers, behavioral health agencies, assisted living communities, housing programs, nonprofits, and other community organizations.
• Generate and coordinate appropriate referrals for Mindful Recovery Center's behavioral health services.
• Conduct outreach visits, presentations, networking meetings, and follow-up with referral partners.
• Plan and participate in community events, health fairs, outreach activities, and educational programs.
• Serve as a community liaison and educate partners about MRC services, eligibility requirements, and referral processes.
• Connect clients with resources such as housing, transportation, medical care, behavioral health services, substance use treatment, and social services.
• Track referrals, outreach activities, community partnerships, and follow-up efforts.
• Assist leadership with program development, community partnerships, and strategies to increase access to services.
• Maintain confidentiality and comply with HIPAA, applicable COMAR requirements, and organizational policies.

Experience Needed,

• Experience in behavioral health, healthcare, case management, community outreach, referral development, or social services preferred.
• Working knowledge of Maryland COMAR regulations, Medicaid, HIPAA, and behavioral health services strongly preferred.
• Strong networking, communication, relationship-building, and organizational skills.
• Experience working with community providers and developing referral relationships.
• Ability to work independently, conduct community outreach, attend events, and professionally represent Mindful Recovery Center.
• Reliable transportation and ability to travel locally for community meetings and outreach activities.

Pay: $42,240.00 - $62,039.37 per year
