Outreach Specialist

Job Overview

We are looking for a Healthcare Outreach Specialist to build and maintain referral relationships with social workers and care managers through phone calls, email, and LinkedIn outreach. The role involves promoting home care services, identifying new referral opportunities, and managing contact lists and follow-ups using Excel and other communication tools.

Schedule : Mon-Fri 9:00 AM - 5:00 PM EST

Responsibilities:

• Conduct consistent weekly outbound calls to social workers and care managers to build and maintain referral relationships
• Proactively identify and connect with new healthcare professionals through LinkedIn and email outreach to expand the referral network
• Communicate the agency's home care services clearly and persuasively to potential referral partners
• Track and organize all contact interactions and follow-up schedules using Excel spreadsheets
• Research and identify new sources of connections and referral opportunities beyond provided contact lists
• Send targeted email communications to care managers and social workers to reinforce phone outreach efforts

Requirements:

• Strong interpersonal and relationship-building skills with the ability to engage healthcare professionals
• Excellent verbal and written communication skills in English
• Proficiency in basic Excel, Outlook, and Gmail
• Experience with LinkedIn for professional networking and outreach
• High level of organization and attention to detail for managing large contact lists
• Ability to work independently and take initiative in identifying new outreach opportunities
