Palliative Care Advanced Practice Practitioner — Full-Time (1.0 FTE)

Faith Palliative Care is seeking a Nurse Practitioner or Physician Assistant to join our team and provide compassionate, individualized care to people living with serious or chronic illness.

Why Join Faith Palliative Care?

Our work centers on helping patients live as well as possible. We provide expert pain and symptom management, support patients and families as they navigate complex medical decisions and develop care plans that reflect each patient’s goals and priorities.

Working in collaboration with patients’ treating providers, our team addresses physical symptoms and emotional concerns while helping patients and caregivers understand their care options. Palliative care can be provided alongside treatment for the underlying illness.

What You’ll Do

  • Provide ongoing care, including assessment, symptom management, and evaluation of treatment effectiveness.

  • Collaborate with patients’ treating providers, palliative care physicians, and other members of the care team.

  • Educate and counsel patients, families, and caregivers about symptoms, treatment options, and goals of care.

  • Participate in the weekend hospital rounding rotation

What You’ll Bring

  • A commitment to compassionate, high-quality palliative care.

  • Strong clinical judgment and the ability to practice independently while collaborating with a team.

  • Flexibility and adaptability across care settings.

  • Strong organizational skills and the ability to prioritize patient needs.

  • Clear, thoughtful communication with patients, families, caregivers, and colleagues.

  • Proficiency with electronic medical records and other clinical technologies.

Education and Experience

  • Current Michigan licensure as a Registered Nurse with Nurse Practitioner specialty certification or as a Physician Assistant.

  • Strong knowledge of clinical care, pharmacology, and pain and symptom management.

  • An understanding of the emotional and psychosocial effects of serious illness and the principles of palliative care.

  • Palliative and/or Hospice care experience strongly preferred.

  • Advanced Certified Hospice and Palliative Nurse (ACHPN®) certification preferred for NP.
