About Us

Palliative Care South East provides palliative care in the southeast metropolitan region of Melbourne. We support people with a life-limiting illness to live well and, in doing so, provide support at home to improve the quality of life of individuals, carers and families. Our specialist palliative care services are provided to people of all ages, free of charge, in the Local Government areas of Cardinia, Casey, Greater Dandenong and part of Kingston.

Position Statement

As our Palliative Care Nurse Practitioner, you'll provide expert clinical leadership and autonomous, NP-led care to clients in their own homes and, in a consultative and clinical advisory capacity, to residents of residential aged care facilities. You'll work alongside our Medical Officers, Advanced Trainee, Nurse Practitioner and specialist interdisciplinary team to deliver comprehensive assessment, diagnosis, treatment and care planning. This role will also support our aims to build the capacity of residential aged care staff and support GPs to deliver a palliative approach across the region.

  • Demonstrated knowledge and understanding of current issues impacting on the provision of Palliative Care services in Australia
  • Demonstrated capability to work autonomously when required
  • Demonstrated abilities in critical thinking, human relations, clinical skills, ethical decision making, problem analysis and systems analysis
  • Demonstrated ability to educate, mentor and support clinical teams to build capability and confidence in delivering high-quality palliative care and influence organisational and cultural change
  • Ability to participate in a rotating on call weekend roster

Why Join PCSE

  • A genuinely varied, autonomous senior clinical role at the heart of a respected specialist palliative care service
  • Work within a values-led organisation — Responsiveness, Empowerment, Collaboration and Kindness — that backs its clinicians
