Palliative Care Nurse Practitioner (NP)

We are seeking a full-time Palliative Care Nurse Practitioner (NP) to join our compassionate team serving Rochelle, IL, and surrounding areas.

• Manage a caseload of palliative patients, providing diagnosis, treatment, medication management, assessments, and consultations.

• Travel to patients’ homes and facilities to deliver personalized, dignified care.

• Develop and expand our palliative care program while maintaining expertise in hospice and palliative practices.

• Collaborate with a multidisciplinary team dedicated to ensuring comfort and peace for patients and their families.

Requirements include a valid APRN license, RN license, and the ability to obtain an NPI number. Preferred candidates have 1-2 years of nursing experience, a controlled substance license, and a collaborative agreement with a Medical Director.
