Duties

• Conduct comprehensive virtual patient assessments focusing on symptom management, psychosocial support, and advanced care planning and transitional care.
• Develop and implement individualized palliative care plans in collaboration with physicians, nurses, social workers, and other healthcare professionals.
• Manage complex symptom control including pain management, dyspnea, nausea, and fatigue using evidence-based practices.
• Prescribe medications within the scope of practice, adhering to DEA regulations and state laws.
• Provide education and emotional support to patients and families navigating serious illness or end-of-life decisions.
• Document all patient encounters accurately in electronic health record (EHR) systems.
• Collaborate with hospice teams, primary care providers, and specialists to coordinate seamless care transitions across settings including home care and inpatient facilities.

Qualifications

• Current Nurse Practitioner license with experience in palliative or hospice & palliative medicine.
• Proven experience in palliative care or hospice settings; experience working with diverse populations including geriatrics, oncology, and chronic illness patients is preferred.
• Strong clinical skills in symptom management such as acute pain management, IV infusion therapy, catheterization, and medication administration.
• Familiarity with EMR/EHR systems like Epic, Cerner or Athenahealth; telehealth experience is highly desirable.
• Knowledge of healthcare diagnosis coding systems including ICD-10 and CPT coding for documentation accuracy.
• Excellent client communication skills with the ability to provide compassionate support remotely.
• Ability to work independently in a remote environment while collaborating effectively with a multidisciplinary team.
• Additional skills such as behavioral health expertise, medical-surgical experience, or ICU background are advantageous.
