About Somerville Homeless Coalition

Somerville Homeless Coalition (SHC) works with individuals and families experiencing homelessness and housing insecurity to help them access stable housing, essential resources, and ongoing support. Our programs include emergency shelter, housing search assistance, street outreach, daytime drop-in services, supportive housing, homelessness prevention, and connections to community resources.

SHC also operates Project SOUP, which provides food assistance through a pantry marketplace, community meals, and grocery delivery for homebound individuals.

Our team works in a dynamic, community-focused environment, providing services ranging from immediate crisis response to long-term housing stabilization. We are committed to helping people build stability, independence, and stronger connections to their communities.

Position Overview

The Supportive Housing Case Manager provides individualized, community-based and home-based case management to individuals and families living in SHC-supported housing programs. This position works closely with formerly homeless clients, including people living with disabilities, mental health challenges, substance use concerns, and other complex needs.

Through regular home visits, advocacy, service coordination, and practical life-skills support, the Case Manager helps clients maintain safe and stable housing, access essential services, and work toward greater independence.
