In this role, you’ll:

• Provide safe, accurate, and timely medication therapy

• Partner with physicians, nurses, and other healthcare professionals to optimize patient outcomes

• Monitor patients and recommend medication adjustments based on clinical needs and laboratory results

• Participate in clinical initiatives including immunizations, discharge planning, transplantation, wellness, and screenings

• Identify and help resolve barriers to patient care and discharge

• Educate patients, staff, and healthcare professionals

• Support pharmacy quality, safety, and workflow initiatives

• Provide leadership and direction to pharmacy technicians and serve as charge pharmacist when scheduled

Qualifications:

• Bachelor’s degree in Pharmacy and less than one year of relevant ambulatory/community pharmacy experience, or equivalent combination of education and experience

• Successful completion of an accredited PGY-1 pharmacy practice residency

• New York State Pharmacist license required upon hire
