Part-Time RN Case Manager

This role requires critical thinking, effective communication, and decisive judgment to build positive relationships and lead others. The incumbent will provide education and resources to support care progression, service utilization, and safety during patient transitions.

Responsibilities include:

• Leading and fostering collaboration within healthcare teams
• Assessing patient needs and coordinating appropriate care levels
• Facilitating safe patient transitions and resource utilization

Qualifications:

• Minimum of one year hospital-based Case Management experience or five years in healthcare
• Graduate of an accredited Registered/Professional Nursing Program
• Basic Life Support (BLS) Healthcare Provider certification upon hire
