Part time/full-time Nurse practitioner

Established Sugar Land primary care practice seeking a Licensed Family Nurse Practitioner (FNP) for a 25–40 hour/week position (in-office and telehealth).

Position Focus:

Primary care and chronic care management with emphasis on lifestyle medicine and prevention.

Responsibilities:

Provide comprehensive primary care (in-office and telehealth)

Manage chronic conditions including hypertension, hyperlipidemia, and diabetes

Conduct monthly 20-minute chronic care management calls

Educate patients on whole food, plant-based nutrition and lifestyle interventions

Document and coordinate ongoing patient care

Qualifications:

Licensed Family Nurse Practitioner (FNP)

Minimum 5 years primary care or bedside clinical experience

Strong interest and working knowledge of whole food, plant-based nutrition

Ability to confidently counsel patients on hypertension, cholesterol, and diabetes

Excellent communication and documentation skills

Bilingual in English and Hindi/Urdu or English and Spanish strongly preferred due to a significant number of Hindi- and Spanish-speaking patients
