POST Rehabilitation and Wellness is a dynamic Outpatient Therapy clinic dedicated to providing top-notch services to our community. POST provides Physical Therapy, Occupational Therapy, and Speech Therapy services. We are seeking a highly organized and proactive Patient Care Coordinator to join our team and contribute to the efficient functioning of our clinic.

Position Overview
The Patient Care Coordinator is the heart of our outpatient therapy clinic. This role is responsible for creating a welcoming, organized, and positive patient experience from the moment a patient contacts our clinic through the completion of their visit. The ideal candidate is a strong culture champion who thrives in a fast-paced, people-focused environment, balances administrative responsibilities with light clinical support, and takes pride in keeping systems and workflows running smoothly.

Key Responsibilities:

Patient Experience & Culture

• Serve as a positive, professional, and friendly first point of contact for all patients, both in person and over the phone.
• Actively promote and model clinic values, culture, and customer service standards.
• Create a welcoming environment that makes patients feel comfortable, supported, and cared for.

Front Desk & Administrative Duties

• Answer incoming phone calls promptly and professionally.
• Schedule, reschedule, and confirm patient appointments to optimize clinic flow
• Collect over-the-counter payments, process transactions accurately, and provide receipts as needed.
• Maintain accurate patient records by organizing paperwork and uploading documentation into the clinic system.
• Ensure all required forms and information are completed, organized, and stored correctly.

Clinic Support & Light Rehab Tech Duties

• During downtime, assist with guiding patients to treatment areas as needed.
• Sanitize and prepare treatment equipment and areas between patient use, following clinic protocols.
• Support therapists and clinic operations as appropriate to ensure smooth patient flow.

Organization & Operations

• Maintain a clean, organized front desk and shared clinic spaces.
• Stay on top of daily tasks, paperwork, and system updates with strong attention to detail.
• Identify opportunities to improve front office efficiency and patient experience.

Qualifications & Skills

High school diploma or equivalent required; healthcare or outpatient clinic experience preferred

Strong interpersonal and communication skills with a patient-first mindset

Highly organized with excellent attention to detail

Comfortable handling payments and basic financial transactions

Ability to multitask, prioritize, and stay calm in a busy clinical environment

Willingness to assist with light physical activity (walking patients, sanitizing equipment)

Proficient with computers and ability to learn scheduling and EMR systems
