Patient Care Coordinator

Overview

As a Patient Care Coordinator, you’ll help patients navigate each step of their care, from scheduling appointments to coordinating records and communicating next steps. You’ll keep information organized, support smooth clinic operations, and deliver thoughtful customer service while protecting patient confidentiality. Your impact: Clear coordination helps patients and care teams stay informed and prepared for appointments.

Duties

• Schedule appointments, manage patient inquiries, and coordinate follow-up care.
• Communicate with patients, providers, and insurance representatives by phone and other channels.
• Maintain electronic health records (EHR) in patient management software, ensuring records are accurate and current.
• Support medical administrative tasks, including document preparation, data entry, typing, and record requests.
• Apply health regulation policies and confidentiality practices when handling patient information.

Qualifications

• Medical office or other office experience; medical administrative support experience is valued.
• Customer service and client communication skills, including professional phone communication.
• Bilingual is a plus
• Familiarity with Microsoft Office, patient management software, and electronic medical records (EMR) or EHR systems.
• Knowledge of medical terminology, health insurance, and health regulation policies is preferred.
• Strong organization, accurate typing, and a commitment to maintaining patient confidentiality.

Pay: $16.00 - $20.00 per hour

Expected hours: 40.0 per week
