Patient Placement Coordinator RN - Full Time

Role Overview This position is a Patient Placement Coordinator RN responsible for evaluating and coordinating patient intake, triaging calls, and facilitating transfers among healthcare facilities. The RN acts as a liaison between physicians, hospitals, and care providers, ensuring timely communication for patient referrals and transfers. He/she employs critical thinking to integrate objective data with patient history for optimal placement. Additionally, the RN provides clinical guidance to the team, supports onboarding and training , and ensures adherence to regulatory standards . The role requires a strong focus on clinical support, communication, and coordination .

Qualifications & Skills

• Associate's Degree in Nursing (ADN) and California RN license required.
• Preferred: Bachelor's Degree in Nursing, 2-5 years of nursing experience, knowledge of Epic and Bed management systems, and call center experience.
• Strong critical thinking, communication, and clinical guidance skills essential.
