Responsibilities

• Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques and patient transfer methods such as Hoyer lifts.
• Monitor patients’ vital signs, observe changes in health status, and accurately document observations to inform care plans.
• Administer medication following prescribed protocols while adhering to HIPAA regulations to ensure confidentiality and privacy.
• Prepare nutritious meals and assist with cooking, ensuring dietary restrictions are respected and residents’ nutritional needs are met.
• Support individuals with dementia, Alzheimer’s, or neurodevelopmental disorders through specialized memory care and behavioral management strategies.
• Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control standards are met.
• Provide compassionate personal care assistance, including toileting, mobility support, and transportation within the community or home setting.
• Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and address individual needs.
• Support residents’ rights by respecting autonomy and promoting a respectful, supportive atmosphere aligned with long-term care standards.

Requirements

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
• Knowledge of patient observation techniques, medication administration, vital signs measurement, and basic first aid/CPR certification.
• Experience working with individuals with autism, developmental disabilities, intellectual disabilities, dementia, or neurodevelopmental disorders is advantageous.
• Ability to perform heavy lifting safely and assist with manual handling tasks such as patient transfers using Hoyer lifts or other equipment.
• Strong communication skills to effectively interact with clients of diverse backgrounds and coordinate with healthcare professionals.
• Familiarity with HIPAA regulations to ensure confidentiality of patient information at all times.
• Demonstrated ability to follow detailed care plans while adapting to individual needs in a caring manner.
• Valid certifications in first aid and CPR are required; additional certifications related to hospice care or geriatrics are a plus.
