Position Responsibilities

• Provide skilled home health care for children and adolescents with diverse medical needs
• Deliver nursing care to patients (ages 0–21) who amedical complex conditions
• Monitor specialized medical equipment, including ventilators and tracheostomy
• Utilize the nursing process: assessment, planning, intervention, implementation, and evaluation
• Assess patient vital signs and intervene appropriately within the community settings.
• Help patient and families understand, manage and navigate their health with confidence in care.
• Accurately document all care provided in a timely manner

Position Requirements

• Graduate of an accredited nursing program
• Current RN license in good standing
• Minimum 1 year of pediatric experience with medically complex patients (required)
• Tracheostomy and ventilator experience required
• PICU experience preferred
