You will support teams in maintaining safe, effective and person-centred care while contributing to safeguarding, quality assurance, clinical governance and service improvement.

The role includes supporting residents with their care needs, medication support under appropriate supervision, monitoring changes in residents' wellbeing, supporting staff practice and contributing to audits, supervision, investigations and improvement activities.

Key Responsibilities

  • Provide flexible clinical support across St Matthews Healthcare services.
  • Support the delivery of safe, effective and person-centred care.
  • Assist residents with personal care, nutrition, communication and other identified care needs.
  • Support medication administration under the supervision of a Registered Nurse or appropriate healthcare professional.

  • Support and supervise junior Health & Social Care Workers as appropriate.

  • Support audits, quality assurance reviews, action plans and service improvement activities.
  • Assist with investigations, information gathering and lessons learnt where required.

Essential Requirements

  • Previous experience in an Assistant Practitioner role for approximately 6 months–1 year.
  • Relevant experience within Health & Social Care.
  • Must be prepared to undertake a Level 5 qualification in Health & Social Care if not already held.
  • Experience of working within a care, healthcare or clinical environment.
  • Strong understanding of safeguarding and the appropriate escalation of concerns.
  • Good knowledge of person-centred care, risk management, confidentiality and accurate record keeping.
  • Strong verbal and written communication skills.
  • Ability to work independently and use initiative.
  • Full driving licence and ability to travel between St Matthews Healthcare locations.
  • Flexible and responsive to operational requirements across multiple services.
  • Able and willing to work across both nursing home and hospital environments.
  • Physically able to undertake the requirements of the role, including mandatory Moving & Handling activities and Basic Life Support (BLS).

About You

You will be confident working across different services and adapting quickly to changing priorities. You should be comfortable working both independently and as part of multidisciplinary teams and be able to communicate concerns clearly and professionally.
