Duties

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to ensure comfort and hygiene.
• Support patients with mobility using proper manual handling techniques and equipment like Hoyer lifts or patient transfer methods.
• Monitor patient health by observing vital signs, noting changes in condition, and reporting findings accurately to healthcare professionals.
• Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary.
• Administer medications following prescribed care plans while adhering to HIPAA regulations to maintain confidentiality.
• Provide transportation assistance for appointments or errands, ensuring safe and timely client transfers.
• Maintain a clean, safe environment by performing light housekeeping duties such as cleaning patient areas and ensuring safety protocols are followed.

Skills

• Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care is highly valued.
• Strong patient observation skills to identify changes in condition or behavior promptly.
• Knowledge of dementia care, Alzheimer's care, memory care, and hospice care best practices.
• Ability to assist with complex patient needs including medication administration, vital signs monitoring, and resident’s rights adherence.
• Familiarity with assisted living environments, nursing homes, home health settings, and long-term care facilities.
• Competence in using patient transfer methods like Hoyer lifts and understanding manual handling safety procedures.
• Excellent communication skills for effective client interaction and teamwork within multidisciplinary healthcare teams.
