Duties

• Assist residents with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using patient transfer methods like Hoyer lifts when necessary
• Monitor residents’ health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare professionals
• Support medication administration in accordance with care plans and HIPAA regulations to ensure safety and compliance
• Prepare nutritious meals and assist with meal consumption while respecting dietary restrictions and preferences
• Maintain a clean and safe environment through regular cleaning duties and ensuring infection control standards are met
• Provide compassionate patient personal care assistance while respecting residents’ rights and dignity
• Support residents with memory care needs, including dementia or Alzheimer’s care, utilizing specialized techniques for safety and comfort
• Assist with transportation to appointments or community activities using appropriate safety procedures
• Document care activities accurately in resident records and participate in developing or updating individualized care plans
• Follow all protocols for manual handling, patient transfer methods, and emergency procedures such as first aid or CPR when needed

Skills

• Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care environments such as group homes or nursing facilities
• Strong knowledge of patient observation techniques, vital signs monitoring, medication administration, and HIPAA compliance
• Excellent communication skills to effectively interact with residents, families, and healthcare teams
• Ability to perform heavy lifting safely and utilize patient transfer equipment like Hoyer lifts effectively
• Familiarity with dementia care strategies including memory support techniques and resident’s rights advocacy
• Skilled in assisting with ADLs, meal preparation, light housekeeping, and maintaining a clean living environment
• Experience working with individuals with autism or developmental disabilities is highly valued
• Knowledge of home & community care practices along with hospice or palliative care experience is advantageous
• Ability to respond calmly in emergency situations through first aid or CPR certification
• Demonstrated compassion, patience, reliability, and a proactive attitude toward caregiving responsibilities
