Responsibilities

• Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and observe patients’ health status, noting vital signs and behavioral changes to inform care plans
• Support medication administration and ensure adherence to prescribed treatments in compliance with HIPAA regulations
• Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking as needed
• Maintain a clean and safe environment through regular cleaning, sanitation, and organization of living spaces
• Utilize equipment such as Hoyer lifts or resident lifts to safely transfer individuals with mobility challenges
• Document care activities accurately in accordance with care plans and healthcare standards, including patient observations and progress notes

Qualifications

• Prior experience working in assisted living, home health, nursing homes, or group homes is highly preferred
• Knowledge of patient observation techniques, vital signs monitoring, and medication administration
• Certifications such as CPR (Cardiopulmonary Resuscitation) and First Aid are required; additional certifications like HIPAA training are a plus
• Experience working with individuals with developmental disabilities, autism spectrum disorder, dementia (including Alzheimer’s and memory care), or hospice care is advantageous
• Ability to perform heavy lifting safely and handle physical tasks involving residents or equipment
• Strong communication skills to collaborate effectively with healthcare teams, residents, families, and social workers
• Compassionate attitude with a commitment to providing respectful, person-centered care
