Responsibilities

• Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to ensure comfort and hygiene.
• Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care plans aligned with healthcare standards.
• Support medication administration following HIPAA guidelines and prescribed schedules to ensure safety and compliance.
• Facilitate patient transfer methods using equipment such as Hoyer lifts and patient transfer techniques to promote mobility and safety.
• Prepare nutritious meals tailored to dietary needs while practicing proper food handling and sanitation procedures.
• Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols.
• Provide transportation assistance for medical appointments or community outings, ensuring safe client mobility and engagement.
• Document care activities accurately in compliance with healthcare regulations such as HIPAA, ensuring clear communication within the care team.
• Engage in patient observation to identify changes in condition or behavior, especially for individuals with dementia, Alzheimer’s, autism, or other neurodevelopmental disorders.
• Support residents’ rights by respecting privacy, dignity, and individual preferences at all times.

Experience

• Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred.
• Demonstrated ability to assist with ADLs and provide personal care support for individuals with disabilities or developmental disabilities.
• Knowledge of patient monitoring techniques including vital signs measurement and dementia or Alzheimer’s care is advantageous.
• Experience working with individuals with neurodevelopmental disorders such as autism or intellectual disabilities supports success in this role.
• Familiarity with care plans development and medication administration procedures is beneficial.
• Certification in CPR and First Aid is required; additional certifications such as Hoyer lift operation or HIPAA training are a plus.
• Ability to perform manual handling tasks including heavy lifting safely while maintaining proper body mechanics is essential for client transfers.
