Personal Care Assistant

• Monitor patients’ health by observing vital signs, noting changes in condition, and reporting findings accurately to healthcare professionals.
• Support medication administration following prescribed care plans while adhering to HIPAA regulations to ensure confidentiality.
• Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking.
• Utilize specialized equipment such as Hoyer lifts or resident’s to ensure safe transfers and mobility assistance.
• Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning patient areas and laundry.
• Document care activities thoroughly in compliance with healthcare standards and assist in updating personalized care plans regularly.

• Proven experience working in home & community care, assisted living, nursing homes, or group homes with a focus on senior or disability care.
• Knowledge of patient observation techniques, vital signs monitoring, medication administration, and dementia or Alzheimer’s care.
• Certification in First Aid and CPR is required; additional certifications such as HIPAA training are preferred.
• Ability to perform heavy lifting safely and operate patient transfer equipment like Hoyer lifts when necessary.
• Strong communication skills with the ability to work effectively with people with developmental disabilities, autism, or other special needs.
• Experience working with individuals requiring memory care or hospice support is highly valued.
