Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting, ensuring their comfort and dignity.
• Monitor patients’ health by observing vital signs, recognizing changes in condition, and reporting findings accurately.
• Support medication administration following care plans and HIPAA regulations to ensure safety and compliance.
• Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary.
• Facilitate patient transfers using manual handling techniques or equipment such as Hoyer lifts, ensuring safety for both clients and staff.
• Maintain a clean, safe environment by performing light cleaning duties and adhering to infection control protocols.
• Document care activities, patient observations, and communication with healthcare providers clearly and thoroughly.

Requirements

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
• Knowledge of patient observation techniques, medication administration, vital signs monitoring, and care planning is essential.
• Familiarity with HIPAA regulations, resident’s rights, dementia care (including Alzheimer’s and memory care), and neurodevelopmental disorders enhances your effectiveness in this role.
• Ability to perform manual handling tasks such as heavy lifting, patient transfers using various methods, and assisting with mobility aids like Hoyer lifts.
• Certification in CPR and First Aid is required; additional certifications in hospice care or autism support are advantageous.
• Strong communication skills for client interaction and teamwork; experience working with individuals with disabilities or developmental challenges is a plus.
• Must be dependable, compassionate, detail-oriented, and committed to providing exceptional personal care assistance.
