Pharmacist

Overview
We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will have a strong background in both clinical and operational pharmacy settings, with a focus on patient care and medication management. This role is essential in ensuring the safe and effective use of medications, providing patient assessments, and collaborating with healthcare professionals to optimize therapeutic outcomes.

Duties

• Dispense medications accurately and efficiently while adhering to all legal and regulatory requirements.
• Conduct comprehensive patient assessments to evaluate medication therapy and ensure optimal outcomes.
• Provide patient education on medication usage, potential side effects, and health management strategies.
• Manage inventory effectively, including ordering, stocking, and maintaining medication supplies.
• Collaborate with healthcare teams to develop individualized treatment plans for patients, particularly in critical care and pediatrics.
• Perform compounding of medications as required, ensuring adherence to aseptic techniques and safety protocols.
• Administer immunizations and manage medication administration processes within the facility.
• Utilize pharmacy software, EMR systems, and EHR systems for accurate record-keeping and patient management.
• Ensure compliance with HIPAA regulations to protect patient confidentiality.
• Participate in pain management strategies and acute care initiatives to enhance patient outcomes.

Experience

• Previous experience in LTC pharmacy or hospital settings is preferred.
• Critical care experience is highly desirable, along with a pharmacy residency or clinical pharmacy experience.
• Familiarity with medical terminology, anatomy knowledge, physiology knowledge, and IV insertion techniques is essential.
• Proven ability in patient service and care management is crucial for success in this role.
