Home Infusion Staff Pharmacist – Syracuse, NY (Direct Hire)

We are seeking a Home Infusion Staff Pharmacist for a Monday–Thursday schedule in Syracuse, NY. This is an opportunity to join a home infusion team supporting complex IV therapies and patient care.

Term of Assignment: Direct Hire

Schedule: Monday – Thursday (8:00 AM – 5:00 PM)

Pay: Starting at $125,000 (annually)

System: CareTend

Job Duties:

• Review infusion orders and patient therapies
• Participate in clinical rounds
• Perform sterile compounding and TPN review
• Maintain patient records and care plans
• Ensure USP and cleanroom compliance
• Train and support staff
• Drive operational improvements
• Evaluate and monitor drug therapy based on the overall needs and clinical conditions of home care patients
• Collaborate with physicians, nurses, and other healthcare professionals regarding patient therapy

Requirements:

• Current New York Pharmacist license in good standing
• Minimum of 2 years of recent acute care experience OR 1 year of recent home infusion therapy experience preferred
• Strong knowledge and experience with intravenous admixture services and intravenous therapy
• Ability to evaluate and monitor drug therapy in collaboration with the interdisciplinary care team
