Physical Therapist Assistant (PTA) – Home Health

Overview
Celestial Home Health Care is seeking a compassionate and dependable Physical Therapist Assistant (PTA) to provide skilled physical therapy services to patients in their homes. The PTA will work under the direction and supervision of a licensed Physical Therapist to carry out established plans of care and help patients improve mobility, strength, safety, and independence.

Responsibilities

• Provide physical therapy treatments according to the established plan of care and under the supervision of a licensed Physical Therapist.
• Perform therapeutic exercises, gait training, transfer training, balance activities, and other interventions within the PTA scope of practice.
• Observe and document patients’ responses to treatment and progress toward established goals.
• Report significant changes in a patient’s condition or progress to the supervising Physical Therapist and appropriate clinical team members.
• Educate patients and caregivers regarding exercises, mobility, safety, and fall-prevention techniques.
• Complete visit documentation accurately and timely in the agency’s electronic medical record system.
• Follow physician/provider orders, the established plan of care, agency policies, and applicable home-health requirements.
• Communicate effectively with the Physical Therapist, nursing staff, physicians/providers, caregivers, and other members of the interdisciplinary team.
• Maintain patient confidentiality and comply with HIPAA requirements.

Qualifications

• Current and unrestricted Florida Physical Therapist Assistant license.
• Graduate of an accredited Physical Therapist Assistant program.
• Home health experience preferred, but not required.
• Strong communication, documentation, and organizational skills.
• Ability to work independently in patients’ homes while following the Physical Therapist’s established plan of care.
• Reliable transportation and ability to travel to patient homes within the assigned service area.

Pay: $50.00 - $55.00 per hour
