Key Responsibilities

• Provide hands-on clinical care and mobility training to patients in their homes.
• Follow and execute individualized plans of care under PT supervision.
• Educate patients and caregivers on safe home exercise programs.
• Complete timely clinical documentation in compliance with regulations.

Qualifications

• Active Texas PTA License (Required).
• Degree from an accredited Physical Therapist Assistant program.
• Valid Driver’s License and reliable, insured vehicle.
• Experience with Homecare Homebase (HCHB) is a major plus (Preferred but not required).
