Chandler Post Acute is a 3 unit Skilled Nursing facility located in Chandler, Arizona! We have a consistent leadership Team, hold each other accountable and are always seeking ways to support one another!

Schedule: 8 Hours Shifts

Job Duties include:

• Provided skilled PT services in the SNF/LTC setting under the direction of a licensed PT
• Assist residents with mobility, balance, strength, and functional activities.
• Collaborate with nurses, therapist, physicians and the interdisciplinary care team.
• Document treatment sessions accurately and timely
• Support residents throughout their rehabilitation and recovery journey.
