Ultimate Therapy is hiring a Physical Therapist to provide individualized rehabilitation services within a skilled nursing setting. This role focuses on improving mobility, reducing pain, and helping residents regain independence.

Key Responsibilities

• Evaluate residents and identify functional limitations
• Create and carry out tailored therapy programs
• Use therapeutic exercises, manual techniques, and modalities
• Monitor progress and adjust care plans as needed
• Educate residents and caregivers on exercises and safety
• Maintain accurate documentation and compliance standards
• Work closely with nursing and interdisciplinary teams

Qualifications

• Active Physical Therapist license
• Strong clinical judgment and communication skills
• Skilled nursing or rehab experience preferred
