What they need
Founding Physical Therapist - Concierge PT & Human Performance
Westport Longevity, Pain & Performance | Westport, CT
Full-time | $125,000-$140,000/year + performance bonus
Practice opens July 2027 | Interviewing now
THIS IS NOT A TRADITIONAL OUTPATIENT PT ROLE
Most outpatient physical therapists spend their day moving between several patients, documenting through lunch, and trying to deliver thoughtful care inside a productivity model built around volume.
This is the opposite of that.
We are building a physician-led interventional pain, longevity, and human performance practice around a small, senior clinical team. Physical therapy will be concierge and cash-based. Most sessions will be private and one-on-one, with the space and autonomy to assess, treat, train, and progress each client properly.
There will be no insurance authorization battles, no crowded clinic floor, no expectation that you supervise multiple unrelated patients while an aide runs their sessions, and no productivity quota based on visits per hour.
You will not simply step into someone else’s protocols, schedule, or gym. You will help design the physical therapy and performance space, build the clinical model, shape the client experience, and take genuine ownership of the department you are helping create.
If you have spent the last few years wanting more autonomy, more ownership, and more time to practice at a high level, this is that job.
ROLES AND RESPONSIBILITIES
Concierge physical therapy. You will provide individualized care for active adults who expect a thoughtful, premium experience. Sessions may include movement assessment, manual treatment, mobility, progressive loading, neuromuscular retraining, strength development, and return-to-activity planning. You will have the time to adjust the session based on what the client actually needs that day.
Preventive care and performance. Not every client will arrive because something hurts. Some will want to move better, improve strength and resilience, return to a higher level of training, or address limitations before they become injuries. You should be comfortable working across the continuum from rehabilitation to performance.
Post-injury and post-operative rehabilitation. You will manage recovery after musculoskeletal injury and surgery, then progress clients beyond basic function toward strength, confidence, and meaningful return to sport, training, work, or daily activity.
Post-procedure rehabilitation. You will work directly with the physician team following injections and interventional pain procedures. This may include rehabilitation after epidural injections, joint procedures, radiofrequency ablation, PRP, and other regenerative treatments. The goal is to connect the procedure to a thoughtful rehabilitation plan rather than treating them as unrelated services.
Strength and performance training. This is not a passive-treatment practice. You should be comfortable coaching foundational lifts, modifying training around injury, programming progressive resistance, and helping clients transition from rehabilitation into independent performance work.
Program ownership. Care will be delivered through premium cash-based programs rather than isolated, insurance-driven visits. You will have meaningful authority over evaluation, program design, session progression, reassessment, and the overall client experience. These programs should be clinically appropriate, goal-directed, and built around enough continuity to produce meaningful results.
Gym and equipment design. You will help design the physical therapy and performance area before the practice opens, including equipment selection, treatment flow, storage, safety, and how the space supports both rehabilitation and strength training. The gym will be intentionally designed for a highly personalized model, generally serving one client and occasionally two compatible clients at a time.
Clinical coordination. You will work closely with our founding physician and the broader clinical team. When a client is not progressing as expected, you will be able to discuss the case directly rather than sending a note into another health system and hoping someone reads it.
Wearing a second hat when the schedule is light. We are a growth-stage practice, and we are honest about what that means. Early in the practice, you may help with movement screenings, performance consultations, community relationships, client education, equipment organization, program development, and other work that helps establish the department.
Building the department. You are early. The evaluation process, clinical pathways, documentation standards, programming model, package structure, gym workflow, and client experience are yours to help shape rather than inherit. As the practice grows, the person in this role is the natural candidate to lead physical therapy and performance.
WHAT WE ARE LOOKING FOR
- Doctor of Physical Therapy degree from an accredited program

- Minimum 2 years of post-licensure clinical experience; 5+ years strongly preferred

COMPENSATION
$125,000-$140,000 annual base salary, plus quarterly performance bonus.
Where you land within the range will depend on clinical experience, advanced training, strength and performance background, leadership ability, and readiness to help build the program from day one.
This is not a commission-only or volume-based position. The base salary is designed to provide stability while the performance bonus allows you to participate meaningfully in the department you help create.
The bonus will reflect a balanced combination of clinical quality, client experience, program completion, department performance, and responsible practice growth.
The upper end of the range is real and reserved for a physical therapist who brings exceptional clinical judgment, strong performance expertise, and the ability to help establish and eventually lead the department.
TIMING
Westport Longevity, Pain & Performance opens in July 2027, and we are beginning conversations now.
We are doing this deliberately. The physical therapist who joins the practice will have a meaningful role in shaping the gym, equipment, clinical model, client experience, and long-term performance program. We would rather take the time to find the right person than fill the position quickly.
If you are successful where you are but frustrated by volume-based care, that is exactly who we want to meet. Early conversations are informal, confidential, and carry no obligation.
If it is a fit on both sides, we will make an offer well in advance of the opening so you can give proper notice and plan your transition.
