Physical Therapist

Job Summary

We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this role, you will be responsible for assessing, developing, and implementing individualized treatment plans to improve patients’ mobility, functionality, and overall quality of life. You will work closely with a multidisciplinary team to deliver exceptional patient care across various settings, including outpatient clinics, inpatient facilities, home health environments, and rehabilitation centers. Your expertise in physiotherapy principles and patient-centered care will help foster recovery and independence for diverse patient populations, including geriatrics, pediatrics, neurology patients, and those recovering from orthopedic or surgical interventions.

Responsibilities

• Conduct thorough assessments of patients’ physical conditions, including mobility, strength, flexibility, and functional limitations.
• Develop comprehensive treatment plans tailored to individual needs, goals, and medical histories.
• Implement evidence-based physiotherapy techniques such as trigger point therapy, manual therapy, therapeutic exercises, and gait training.
• Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate patient care strategies.
• Manage clinical documentation standards accurately within electronic health records (EHR) systems for all patient interactions and treatment progress.
• Educate patients and their families on care plans, home exercises, injury prevention strategies, and community resources to support ongoing recovery.
• Assist in discharge planning by evaluating patient progress and recommending appropriate follow-up care or referrals to outpatient or home health services.

Experience

• Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospital settings, or home health environments.
• Demonstrated knowledge of physiology, anatomy, medical terminology, and neurological assessment techniques.
• Experience working with geriatric patients as well as pediatric populations in outpatient rehabilitation centers or home & community care settings.
• Familiarity with outpatient rehabilitation procedures including orthopedics and neurology treatments.
• Proficiency in managing electronic health records (EHR) or electronic medical records (EMR) systems with attention to detail in documentation standards.
• Hands-on experience with treatment plan development that incorporates discharge planning and patient rehabilitation goals.
• Ability to communicate effectively with clients of diverse backgrounds while providing compassionate patient care across various clinical environments such as hospitals or outpatient clinics.

Pay: $48.42 - $58.31 per hour

Expected hours: No less than 4.0 per week
