Job Overview

We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. Duties

• Conduct comprehensive patient evaluations to determine physical therapy needs and develop individualized care plans aligned with clinical standards.
• Implement evidence-based treatment strategies such as manual therapy, trigger point therapy, and therapeutic exercises tailored to each patient's condition.
• Collaborate closely with healthcare professionals including physicians, nurses, and occupational therapists to coordinate care and optimize patient outcomes.
• Manage electronic health records (EHR) systems accurately to document assessments, treatment progress, discharge planning, and compliance with medical terminology standards.
• Assist in discharge planning by preparing patients for transition back to home or community settings with appropriate follow-up care instructions.
• Work with diverse populations including geriatric patients, pediatrics, neurology cases, orthopedics, and patients recovering from surgery or injury.
• Educate patients and their families on treatment goals, physiotherapy techniques, and home exercise programs to promote ongoing recovery and independence.

Skills

• Strong knowledge of physiology, anatomy, and medical terminology relevant to physical therapy practices.
• Proven experience in outpatient rehabilitation centers, inpatient hospital units, or home health environments.
• Skilled in developing effective treatment plans that address individual patient needs across various specialties such as orthopedics, neurology, geriatrics, and pediatrics.
• Familiarity with clinical documentation standards and EMR/EHR systems for accurate record-keeping.
• Ability to communicate clearly and empathetically with patients of all ages while fostering trust and motivation.
• Experience in discharge planning and working collaboratively with healthcare teams to ensure seamless patient transitions.
• Working knowledge of outpatient clinic operations, inpatient care protocols, and home & community care practices.
