Responsibilities

• Provide skilled home health PT evaluation and treatment
• Develop individualized plans of care to promote safety, independence, and functional mobility
• Communicate effectively with patients, caregivers, and interdisciplinary teams
• Complete documentation in a timely and compliant manner

Requirements

• Current California PT License
• Home Health experience preferred (training/support available)
• Reliable transportation and commitment to patient-centered care
• Strong communication and time management skills

Pay: $150,000.00 - $210,000.00 per year
