Whether working in outpatient clinics, hospitals, or home health settings, you’ll collaborate with multidisciplinary teams to deliver exceptional care rooted in evidence-based practices. This position is negotiable as an employee with a set hourly rate versus an independent contractor with negotiable payment.

Responsibilities

• Conduct comprehensive patient assessments to determine physical therapy needs, including evaluating mobility, strength, and functional limitations.
• Develop and implement individualized treatment plans aligned with clinical best practices and patient goals.
• Deliver hands-on therapy interventions such as therapeutic exercises, manual therapy, trigger point therapy, and modalities to facilitate recovery.
• Manage discharge planning processes and coordinate care with healthcare professionals to ensure seamless patient transitions across settings.
• Document all patient interactions accurately in electronic health records (EHR) systems following clinical documentation standards.
• Collaborate with physicians and other team members to optimize treatment outcomes.
• Educate patients and their families on injury prevention, home exercise programs, and community resources for ongoing support.

Skills

• Strong knowledge of physiology, anatomy, orthopedics, neurology
• Skilled in discharge planning and care plan development
• Familiarity with medical terminology and physiotherapy techniques including trigger point therapy.
• Ability to effectively communicate with clients of diverse backgrounds and explain treatment plans clearly.
• Experience managing electronic health records (charm Health EHR) systems while adhering to clinical documentation standards.

Pay: $40.00 - $59.00 per hour
