Physical Therapist

Overview

We are seeking a dedicated and highly skilled Physical Therapist to join our healthcare team. The ideal candidate will possess a comprehensive understanding of physical therapy practices, with experience in outpatient, inpatient, and community care settings. This role offers an opportunity to deliver patient-centered rehabilitation services, develop individualized treatment plans, and collaborate effectively with multidisciplinary healthcare professionals. Responsibilities

• Assess patients’ physical conditions by reviewing medical histories, conducting examinations, and utilizing clinical documentation standards.
• Develop and implement personalized treatment plans that address specific patient needs, including orthopedics, neurology, and geriatrics.
• Provide therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to promote mobility and reduce pain.
• Manage discharge planning processes to ensure seamless transition of care from outpatient settings to a home environment.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate comprehensive patient care strategies.
• Maintain accurate electronic health records (EHR) and ensure compliance with clinical documentation standards.
• Educate patients and their families on treatment procedures, post-treatment exercises, and injury prevention techniques.
• Participate in ongoing professional development activities to stay current with advancements in physical therapy practices and physiology knowledge.

Experience

• Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or inpatient rehabilitation centers.
• Demonstrated expertise working with diverse patient populations including geriatrics, pediatrics, and neurological cases.
• Strong background in discharge planning, care plan development, and patient rehabilitation strategies.
• Familiarity with electronic health records (EHR) systems such as EMR platforms for documentation and treatment tracking.
• Knowledge of clinical documentation standards and medical terminology relevant to physical therapy.
• Prior involvement in outpatient rehabilitation centers or hospital settings that emphasize patient care excellence.

Pay: $80,000.00 - $105,000.00 per year
