Job Summary

We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. In this role, you will be responsible for assessing, planning, and implementing personalized treatment plans to improve patient mobility, reduce pain, and enhance overall quality of life. You will work closely with patients across various settings—including outpatient clinics, inpatient hospitals, home health environments, and rehabilitation centers—to deliver exceptional care rooted in physiotherapy principles. Your expertise will help patients recover from injuries, surgeries, or manage chronic conditions while adhering to clinical documentation standards and collaborating with multidisciplinary healthcare professionals.

Duties

• Conduct comprehensive patient evaluations to determine physical therapy needs, including assessing physiology, anatomy, and medical history.
• Develop and implement tailored treatment plans that incorporate therapeutic exercises, manual therapy techniques such as trigger point therapy, and modalities suitable for diverse patient populations.
• Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare providers to ensure cohesive patient care strategies.
• Manage discharge planning processes by preparing care plans that facilitate smooth transitions from hospital to home or community settings.
• Document all patient interactions accurately within electronic health records (EHR) systems while maintaining compliance with clinical documentation standards.
• Provide patient education on injury prevention, treatment procedures, and home exercise programs to promote ongoing rehabilitation outside clinical settings.
• Assist in acute care scenarios by applying your inpatient experience to stabilize patients and support their recovery journey through evidence-based physiotherapy interventions.

Experience

• Proven practicing experience in a physical therapy role within outpatient clinics, inpatient hospitals, or home health environments.
• Demonstrated expertise in outpatient rehabilitation center settings with familiarity in orthopedics, neurology, pediatrics, geriatrics, and geriatrics-specific care.
• Strong knowledge of physiology, anatomy, medical terminology, and treatment modalities relevant to diverse patient needs.
• Experience working with geriatric patients and managing complex cases involving chronic conditions or post-surgical recovery.
• Proficiency in electronic health records (EHR) management systems such as EMR platforms for accurate documentation of patient care activities.
• Skilled in developing effective treatment plans that incorporate physiotherapy techniques like trigger point therapy and functional mobility exercises.
• Ability to communicate clearly with clients of all ages and backgrounds while fostering a compassionate environment for patient rehabilitation.

Pay: $50.00 - $55.00 per hour
