Physical Therapist

Optimize Home Health is looking for a Physical Therapist to join our growing home health team.

We are looking for a PT who enjoys working independently and wants flexibility with their schedule and caseload. You will provide physical therapy services to patients in their homes and have flexibility in choosing your availability and the areas you are comfortable covering.

Previous home health experience is helpful, but it is not required. If you are new to home health, we are happy to provide training and help you become comfortable with home health documentation, scheduling, and patient care.

Responsibilities:

• Complete physical therapy evaluations and re-evaluations
• Develop individualized plans of care and treatment goals
• Provide skilled physical therapy services in the patient's home
• Address mobility, strength, balance, gait, transfers, and functional safety
• Educate patients, families, and caregivers
• Complete required documentation in a timely manner
• Communicate with physicians and other members of the care team
• Complete discharge assessments when appropriate
• Coordinate care with nursing, OT, and other disciplines involved with the patient

Requirements:

• Current California Physical Therapist license
• Valid driver's license
• Reliable transportation
• Ability to travel locally to patients' homes
• Good communication and documentation skills

New graduates and Physical Therapists without previous home health experience are welcome to apply.

Pay: $140,000.00 - $200,000.00 per year
