Physical Therapist

A Blessing Hand Home Health is looking for a Per diem Physical Therapist to provide therapy in home. We are looking for a caring Physical Therapist to help our patients.

Pay rates depend on experience and dependent on the type of assessment scheduled.

Essential Job Functions/Responsibilities

Provide physical therapy services to patients according to written physician's plan of care.

Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Formulates treatment plan based on the patient's medical history and in-person evaluations, as well as referrals and notes submitted by other physician's and medical experts.

Evaluate home environment for safety and help make modifications if necessary.

Complete Oasis assessments.

Instructing patients and families/caregivers in the use and care of therapeutic exercises.

Determining priority needs for physical therapy.

Reporting to physician the patent's reaction to treatment or changes in condition. Provide education and at-home techniques to the patient and their family to ensure full recovery.

Monitors and documents patient's progress for discharge.

Pay: $65.00 - $110.00 per hour

Work Location: On the road
