PRN Home Health Physical Therapist - South / Southwest Chicago Suburbs

Versed is partnering with Loyal Home Health to hire a PRN Home Health Physical Therapist for a local, flexible caseload across the South and Southwest Chicago suburbs.

Quick snapshot:

• $75/hour starting compensation
• 10-12 visits per week currently available
• Primary territory: Joliet, Plainfield, Orland Park, Lockport, and Lemont
• Flexible PRN schedule that can fit around another clinical role
• Home health caseload focused mainly on orthopedic and post-acute patients

This is a strong fit for an Illinois-licensed PT who wants consistent PRN visit volume without taking on a full-time role. You will manage your own patient schedule and route while working with a collaborative home health team.

The territory is designed to stay geographically practical whenever possible. Additional coverage may include Homer Glen, New Lenox, Mokena, Frankfort, Tinley Park, Romeoville, Bolingbrook, Shorewood, and nearby communities.

What you'll do

• Provide skilled physical therapy services in patients' homes
• Complete evaluations, reassessments, treatments, and other skilled PT visits
• Complete Start of Care visits and admissions as needed, including PT-only cases
• Develop plans of care focused on mobility, strength, safety, and functional independence
• Document accurately and on time, including OASIS and HomeCare HomeBase (HCHB) documentation
• Coordinate care with nursing, therapy, physicians, patients, and caregivers
• Educate patients and caregivers on safety, mobility, home exercise programs, and recovery goals

What we're looking for

• Licensed Physical Therapist in Illinois
• Prior home health experience strongly preferred
• Comfortable working independently in a field-based setting
• Experience with orthopedic and post-acute patients preferred
• Experience with Start of Care visits, OASIS, and HCHB preferred
• Strong clinical judgment, communication, and time management skills
• Reliable, organized, and responsive with patients and clinical partners
