Physical Therapist

We are looking for a committed and patient-focused Physical Therapist to develop, recover or maintain the daily living and work skills of people with physical, mental or developmental conditions. The physical therapist contracted through the organization is responsible for the implementation of standards of care for physical therapy services for Home Health and Hospicepatients.

Pay:

• Per Diem: $120-130 Per PT evaluation/ $90 per follow up visit
• Mileage Reimbursement

Responsibilities:

• Evaluate and assess patients' physical conditions, including their medical history and any relevant test results
• Develop and implement individualized treatment plans based on patients' needs and goals
• Provide hands-on therapy techniques, such as trigger point therapy, to relieve pain and improve mobility
• Educate patients and their families on exercises and techniques to promote recovery and prevent further injury
• Monitor patients' progress and adjust treatment plans as necessary
• Collaborate with other healthcare professionals to ensure comprehensive patient care
• Maintain accurate and up-to-date patient records

Skills:

• Strong knowledge of anatomy, physiology, and medical terminology
• Experience working in a hospital or acute care setting is preferred
• Specialization in pediatrics is a plus
• Excellent communication and interpersonal skills to effectively interact with patients, their families, and other healthcare professionals
• Ability to assess patients' needs and develop appropriate treatment plans
• Strong problem-solving skills to address any challenges or complications that may arise during therapy sessions
