Duties

• Conduct comprehensive patient evaluations to determine functional limitations and develop tailored treatment plans.
• Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, neurology interventions, and geriatrics care.
• Collaborate closely with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate patient care and discharge planning.
• Manage electronic health records (EHR) systems accurately to document assessments, treatment progress, and clinical notes in accordance with documentation standards.
• Facilitate patient rehabilitation sessions focusing on mobility improvement, pain management, and functional independence.
• Educate patients and their families on treatment goals, home exercises, and community resources to support ongoing recovery.
• Assist with outpatient care transitions by developing comprehensive care plans that meet clinical needs across settings.

Requirements

• Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
• Proven experience practicing in a physical therapy role within acute care, outpatient clinics, hospital settings, or home health environments.
• Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques applicable to diverse patient populations including pediatrics and geriatrics.
• Experience with home health and outpatient rehabilitation centers or inpatient units is highly desirable.
• Familiarity with EMR (Electronic Medical Records) or EHR systems for efficient documentation management.
• Ability to develop effective treatment plans based on patient assessments and clinical guidelines.
• Excellent client communication skills to educate patients clearly about their care plans and progress.
