MIK-BEN Helping Hands LLC is seeking a compassionate and dependable Physical Therapist (PT) to provide home health physical therapy services to patients in nearby Houston and surrounding areas.

Responsibilities:

• Evaluate patients’ mobility, strength, balance, and functional abilities
• Develop and implement individualized physical therapy plans of care
• Provide therapeutic exercises, gait training, and balance and mobility training
• Monitor patient progress and adjust treatment plans as appropriate
• Coordinate care with physicians, nurses, and other therapy professionals
• Educate patients and caregivers on home exercises, safe mobility, and fall prevention
• Complete accurate and timely evaluations, visit notes, and discharge documentation

Requirements:

• Active Texas Physical Therapist license in good standing
• Home health experience preferred
• Knowledge of Medicare guidelines preferred
• Strong clinical and documentation skills
• Reliable transportation and valid driver’s license
• Current CPR certification
