Physical Therapist (PT)

Coral Reef Home Health Solutions is seeking a compassionate and motivated Physical Therapist (PT) to provide skilled home health services to private-duty patients throughout Broward County, Florida.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized plans of care.
• Provide skilled physical therapy interventions in accordance with the established treatment plan.
• Evaluate patients' functional abilities, mobility, strength, balance, and safety needs.
• Modify treatment plans based on patient progress and clinical findings.
• Educate patients and caregivers on therapeutic exercises, fall prevention, mobility, and home safety.
• Supervise Physical Therapist Assistants (PTAs) as required by Florida regulations and agency policies.
• Maintain accurate, timely, and complete clinical documentation.
• Collaborate with physicians and the interdisciplinary team to ensure quality patient care.
• Comply with all agency policies, procedures, and applicable state regulations.

Qualifications

• Current Florida Physical Therapist (PT) License.
• Current CPR Certification.
• Valid driver's license and reliable transportation.
• Professional liability insurance.
• Excellent communication, assessment, and organizational skills.
• Home health experience is preferred but not required.
• Ability to work independently and manage patient caseloads efficiently.
