What You'll Do

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services in accordance with physician orders and regulatory guidelines
• Collaborate with physicians, nursing staff, therapy team members, residents, and families
• Monitor patient progress and modify treatment plans as needed
• Complete timely and accurate documentation
• Educate residents and caregivers on mobility, safety, and recovery strategies
• Support residents in achieving maximum independence and quality of life

What We're Looking For

• Current California Physical Therapist (PT) license
• Graduate of an accredited Physical Therapy program
• Strong clinical, communication, and documentation skills
• Skilled nursing or post-acute experience preferred, but not required
• A patient-centered mindset and passion for helping others succeed
