Physical Therapist

Job Overview.

We are practioner owned practice. The ideal candidate will provide comprehensive patient care across various settings, including inpatient, outpatient, home health, and community care. This role involves developing and implementing personalized care plans to improve mobility, reduce pain, and promote overall well-being. The Physical Therapist will utilize their expertise in physiology, anatomy, and medical terminology to deliver evidence-based physiotherapy services tailored to diverse patient populations such as pediatrics, geriatrics, orthopedics, and those recovering from acute care or hospital stays.

Responsibilities

• Assess patients’ physical conditions through detailed evaluations and develop individualized care plans.
• Implement therapeutic interventions including trigger point therapy, manual therapy, and exercise programs to enhance mobility and function.
• Coordinate discharge planning and ensure seamless transition of care between hospital, home, and community settings.
• Monitor patient progress and adjust treatment strategies accordingly to optimize outcomes.
• Educate patients and their families on exercises, injury prevention, and health maintenance techniques.
• Document all patient interactions accurately using PROMPT in compliance with healthcare regulations.
• Collaborate with multidisciplinary teams including physicians, nurses, and other healthcare professionals to deliver holistic patient care.
• Stay current with advancements in physiotherapy, orthopedics, geriatrics, and related fields to ensure the highest quality of care.

Experience

• Proven experience in outpatient, or hospital environments is preferred.
• Demonstrated expertise in care plans, discharge planning, and working with diverse populations such as adult and geriatrics.
• Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice.
• Experience utilizing various treatment modalities including trigger point therapy and orthopedic rehabilitation techniques.

Pay: $45.00 - $55.00 per hour
